63.2 Licensure and Registration Procedures

An unannounced renewal inspection was completed on 11/15/18 from 10:45 until 12:40pm. At the time of entrance, there were 5 children in care for a total of 15 points with the provider. A sample of 5 children's records and 2 provider/household member records were reviewed. Children were observed playing with age appropriate toys and eating lunch. Hand washing, toileting and infant feeding and diapering were also observed. The provider does not currently administer medication or provide transportation at this time. There have been no injuries to children in care since the last inspection. First aid and emergency supplies, documentation of emergency practice drills and required family day home postings were reviewed. Two violations in the areas of administration and emergencies are identified on the violation notice and were discussed with the provider during the exit interview.

Description: Based on observation and interview, the provider did not ensure that the family day home shall have a written emergency preparedness and response plan that includes all the required elements. Evidence: 1. The emergency preparedness and response plan was not available for review at the time of the inspection. 2. The provider confirmed that she had redone the emergency preparedness and response plan but that it could not be located during the inspection.

Plan of Correction: The provider will locate the emergency preparedness and response plan and will post it in the family day home and will put an additional copy in the licensing folder.

Standard #: 22VAC40-111-60-B
Description: Based on record review and interview, the provider did not ensure that all children's records contain all required information. Evidence: 1. The record for child 1 does not contain documentation of proof of the child's age and identity. 2. The record for child 2 does not contain the addresses of two designated persons to contact in case of an emergency if the parent cannot be reached. 3. The provider confirmed that the records for child 1 and child 2 do not contain all the required information.

Plan of Correction: The provider will obtain the proof of identity for child 1 and the emergency contact addresses for child 2 from the parents by the end of the week.
