Comments:
An unannounced monitoring inspection was conducted this date. A total of 11 children in care, 2 of which are related to the provider(29pts) with Provider and 2 assistants. When the inspector arrived the children had completed lunch, was transitioning to hand washing and nap time. Staff observed monitoring children while napping. Observation of the crib area, bathroom and both program rooms was conducted. Walk thru of entire home with provider was completed. The children were proud of their art projects they completed earlier today which were painted paper plate turkeys for the holidays. No medications on site today and no identified food allergies. A total of 3 children's records and all staff records and household records were reviewed. Previous violations from the last inspection were reviewed and corrective actions verified. Standard #: 22VAC40-111-100-A
Description: Based on child record review, 1 child's record did not have documentation of a physical exam. Evidence: Child #2 began care on 9/17/18, a physical exam was not obtained within 30 days of the first day of attendance.

Plan of Correction: We will request a copy of the physical exam today and maintain it in the record.

Standard #: 22VAC40-111-340-C
Description: Based on observation of the bathroom, paper towels were not maintained. Evidence: The bathroom located on the lower level of the home of the programming area does not have paper towels available.

Plan of Correction: We will place paper towels within reach of the children to use.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation of the home, a required posting was not found. Evidence: The most recent violation notice from the March 2018 inspection was not posted.

Plan of Correction: We will post today's inspection.
