Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on November 19, 208 from 10:45 a.m. through12:40 p.m. There were ten children and two resident children in care along with the provider and three assistants. Points totaled forty-nine. No medication is being administered at this time. Five children's records were reviewed. The record for the provider, and three assistants were reviewed. Areas of non-compliance may be found on the violation notice. Areas observed were: infant feeding, lunch, movement, hand washing/diapering procedures, and nap. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. This family day home is well equipped with a variety of age/stage materials for the children in care. Standard #: 22VAC40-191-40-D-1-b
Description: Based on record review, the provider failed to obtain a finger print check on one new employee before beginning to work. Evidence: 1. Staff #2 did not have documentation of obtaining a fingerprint check before working with children. 2. Staff #2's date of hire was documented on 10/29/2018. 3. The provider confirmed fingerprint checks had not yet been obtained.

Plan of Correction: Staff #2 scheduled an appointment during the inspection to obtain fingerprints. The appointment is scheduled for November 20, 2018.
