A Subsidy Health and Safety inspection was conducted on 11/21/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 8:55am-12:15pm. There were 6 children (14 points) in the direct care and supervision of 3 caregivers. A sample of 4 children?s records and 3 caregiver's records were reviewed. The children were observed playing, singing and dancing. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-160-B Description: Based on record review, the provider did not maintain a record of daily attendance which recorded the departure of each child. Evidence: The attendance kept did not record the departure of each child. Plan of Correction: I will print out a form.
Standard #: 22VAC40-665-230-A Description: Based on record review, each caregiver had completed the Virginia Preservice Training. Evidence: Caregive B had not completed the Virginia Preservice Training. Plan of Correction: She will complete it by the end of December.
Standard #: 22VAC40-665-410-A-3 Description: Based on record review, lockdown procedures were not practiced annually. Evidence: Lockdown procedures were not practiced for 2017. Plan of Correction: I will practice one for 2018 on the 26th.
