Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the possibility of the Family Day Home (FDH) participating in the subsidy program. While the FDH is not currently participating in this program, the Provider indicated that the paperwork was in the process of being completed. Some discussion took place on a few of these subsidy standards.

The following inspection took place between the hours of 8:30 am through 11 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced renewal inspection there were a total of 8 children (20 points) with 3 caregivers. A total of 8 children?s files, 3 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: drop-off, breakfast, hand washing, bathroom breaks, song, dance, learning animals & colors, games, music, large motor activities in the FDH activity room, and other organized activities. Standard #: 22VAC40-111-170-A
Description: Based on review, 1 of 2 household member's files reviewed indicated that the Provider did not obtain for each adult household member a current Report of Tuberculosis Screening (TB) form (published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form). Evidence - 1. On the date of inspection (11/13/2018), current TB documentation was not available for household member #2. 2. Member #2 has recently turned 18 years of age and now falls under the definition of an adult household member. 3. TB documentation is on file. However, it is not current; being older than two years in age with the date - 03/31/2015.

Plan of Correction: Updated TB documentation shall be obtained for household member #2 and placed on file.
