A complaint was received on 11/2/18 and initiated on the same day regarding personnel. The investigation concluded on 11/8/18. During the investigation three assistants and the licensee were interviewed. Assistant records were reviewed. The information gathered during the investigation supports the allegation. Therefore, the complaint is valid. A single violation regarding personnel was identified and appears on the violation notice.
Standard #: 22VAC40-111-150-C Complaint related: No Description: Based on interview the provider failed to ensure that an assistant 18 years or older is not left alone with children in care for more than two hours per day. Evidence: 1 - Assistant 1, 2 and 3 stated that the licensee is away from the licensed family day home each day 7:30am and 12noon. - Assistant 1 stated t that the licensee leaves the home daily at approximately 7:30am. Assistant 1 stated that she works in the home from 7am to 10:30am daily. - Assistant 2 stated she works in the home from 8am to 12noon daily. Assistant 2 also stated that she does not see the licensee until after 12 noon. - Assistant 3 stated that she is present providing care in the home from 10:30am to 3:00pm and that the licensee arrives around 12 noon each day. - The licensee stated that she is away from the home daily from approximately 7:30am until 12 noon. During that time she is taking her son to school and she has a job in the public schools. The position with the public schools began in September 2018. Plan of Correction: [ ] is the assigned substitute when the licensee is away. See college credits attached.
