An unannounced renewal inspection was conducted on 11/29/18 from 10:00am until 12:30pm. At the time of entrance, there were 11 children in care for a total of 22 points with the provider and one assistant. A sample of 5 children's records and 5 provider/household member/assistant records. Children were observed participating in morning learning activities, reading and eating lunch. Hand washing, toileting and lunch preparation procedures were also observed. The provider does not administer medication to children in care and there have been no injuries to children since the last inspection. The emergency preparedness plan, first aid and emergency supplies and required family day home postings were reviewed. One violation in the area of physical health of caregivers and household members is identified on the violation notice and was discussed with the provider during the exit interview.

Description: Based on record review and interview, it was determined the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening. Evidence: 1. The most recent tuberculosis screening for staff 1 is dated 7/6/16. 2. The provider confirmed that the tuberculosis screening for staff 1 is overdue.

Plan of Correction: The provider stated staff 1 is a substitute caregiver and has not been used in some time. Staff 1 will obtain an updated tuberculosis screening next week, which will be placed in staff 1's record.
