The licensing inspector conducted an unannounced monitoring inspection on 11/29/2018 from 2:33pm to 4:50pm to review supplemental health and safety requirements. The children were napping upon the inspector?s arrival. Today, there were six children, totaling 14 points, present with two caregivers. Interviews were held with the caregivers throughout the inspection. The provider does administer medication; however, they did not have any being given at this time. The provider does not provide transportation. The provider does have a pet. Five children?s records and two caregiver/household member records were reviewed during the inspection. Last emergency drill: 11/16/2018 Last shelter-in-place drill: 09/14/2018 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not ensure that two of five children?s records contained documentation of the child's physical examination prior to the child's attendance or within thirty days after the first day of attendance. 1. The record of child #1 (start date: 9/04/18) had documentation of a physical record dated by a physician on 11/20/2018. 2. The record of child #2 (start date: 10/16/17) had documentation of a physical record dated by a physician on 1/23/2018. Plan of Correction: I will make sure I don't overlook the physical and immunization records.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not ensure that two of five children's records contained documentation of immunization records before attendance. Evidence: 1. The record of child #1 (start date: 9/04/18) had documentation of an immunization record dated by a physician on 11/20/2018. 2. Standard #: 22VAC40-111-330-A Description: Based on observation, the provider did not ensure to obtain a landline telephone, excluding a cordless or cell phone, which should be available, operable, and accessible during the family day home's hours of operation. Evidence: The provider did not have an operable landline telephone in the home. Plan of Correction: I will go to the store and purchase a landline phone.
