Comments:
An unannounced inspection has been completed. There were 5 children and a resident child (infant- 3 years) with the Provider for a total of 16 points. There were children sleeping and one awake two year old child. The Provider has attended various training s that totaled over 20 hours in annual training. Provider's CPR/First Aid and MAT training are current. Please call with questions 703-479-4679

Standard #: 22VAC40-111-800-A
Description: Based on review of 2 children's files, the Provider did not include the relocation address on the Emergency preparedness plan. Evidence: 1. Two files reviewed and the Emergency plan did not include the address in the event Provider has to relocate in an emergency.

Plan of Correction: I will record the address.

Standard #: 22VAC40-111-830-D
Description: Based on record review, the Provider did not record the shelter in place drills. Evidence: 1. Fire/evacuation drills are recorded but shelter in place are not. 2. Provider said they do practice and go to the bathroom but she did not write them down.

Plan of Correction: I will write them down when we do them.

Standard #: 22VAC40-111-240-A
Description: Based on observatin of home, the provider has not maintained some areas of home. Evidence: 1. Door frames have visible chipped and peeling paint.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-111-450-A
Description: Based on observation, the glass sliding door is not clearly marked with decorative objects (pictures, art work, or decals) at the eye level of children. Evidence: 1. Glass door has blinds but is partially open and exposes glass.

Plan of Correction: I will decorate door at child's level.

Standard #: 22VAC40-191-40-D-1-a
Description: Based on interview and record review, household member #2 does not have a Central Registry Check. Evidence: 1. Household member moved in in the summer and has a finger print search. 2. The Provider said she didn't think she needed CPS check because of finger print check and tat she has not done it.

Plan of Correction: I will have her finish it today and send out today. I will send you a copy when it comes in.
