Comments:
An unannounced renewal inspection was conducted today between 10:20am and 11:30am. 5 children (17pts) were present with 2 caregivers. Ratios were in compliance. The physical space, 3 children's records, caregiver/assistant/household member records, evacuation drills, the menu and attendance records were reviewed. Required documents were posted. The space was organized and contained an abundance of developmentally appropriate materials. Children were observed in free play and washing hands to prepare for lunch. Interactions between the caregivers and children were positive. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on records review, 2 out of 3 children's records did not have a policy number of the child's medical insurance, if applicable. Evidence: Child #1 and Child #2 both had the name of the insurance company but not the policy number.

Plan of Correction: I will ask parents for the information today.

Standard #: 22VAC40-111-210-A
Description: Based on interviews and documentation review, caregivers did not obtain 16 clock hours of training annually. Evidence: In the past 12 months, the Provider received 11 clock hours of training and the 2 assistants received 5 hours each.

Plan of Correction: We will get caught up with training and stay on top of scheduling the training in the future.
