- Technical Assistance:
Suggest obtaining additionally adhesive tape for the first aid kit. The roll of tape is low. The new/revised renewal application is located on the DSS web site.
An unannounced monitoring inspection was conducted on 11/28/2018 from 12:40pm - 2:45pm. At the time of entrance there were seven children in care with two caregivers. The ages of children counted for a total of 16 points. Children were observed napping. Three preschool children were observed playing with toys in the playroom with the assistant. Records were reviewed for the seven children in care and both caregivers. The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.
Standard #: 22VAC40-111-780-B Description: Based on record review, the provider failed to obtain an annual review with the parent for the emergency contact information and obtain the parent's signed acknowledgment of the review. Evidence: 1. The record for child 1 was last reviewed by the parent on 09/13/2017. 2. The record for child 2 was last reviewed by the parent on 09/13/2017. 3. The record for child 3 was last reviewed by the parent on 12/01/2016. 4. The record for child 4 was last reviewed by the parent on 09/15/2017. Plan of Correction: Each parent will be asked to review their child's record at pick-up this afternoon. The parent will be asked to sign the record verifying the review.
Standard #: 22VAC40-111-280 Description: Based on observation, the provider failed to ensure that potentially poisonous substances shall be stored in areas inaccessible to children. Evidence: 1. A can of aerosol spray disinfectant and a can of aerosol spray air freshener was observed placed behind the television on the television stand. a. This area is located next to the parent sign in table and is accessible to the preschool age children in care. Plan of Correction: These products will be placed in an area inaccessible to children.
