At the time of inspection, inspector collected the renewal application from the provider. Inspector provided resources regarding emergency preparedness plan, injury report and topical creams for the children.
- Comments:
-
An unannounced renewal inspection was conducted today from 9:30am-11:10am. There were 4 children plus the provider's 2 children totalling 17 points directly supervised by 2 staff. The home, 2 staff records, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, playing with cars, trucks, and dinosaurs and eating snack. Snack today was crackers and yogurt. Infants were observed doing tummy time and crawling freely. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-800-A Description: Based on record review, the provider did not have a written emergency preparedness plan. Evidence: When the written emergency preparedness plan was requested, the provider stated it was not yet completed. Plan of Correction: We will complete the plan by Friday, 12/7/2018.
Standard #: 22VAC40-111-830-C Description: Based on observation, the provider did not maintain documentation of monthly drills. Evidence: The November 2018 drill was not documented. The provider stated it was completed. Plan of Correction: We will ensure to document drills.
Standard #: 22VAC40-111-60-B Description: Based on record review, the provider did not maintain complete records for each enrolled child. Evidence: 1) Child 1 is missing doctor's name and phone number, 2 emergency contacts, medical insurance and policy number. (Child 1, start date 11/26/18) Plan of Correction: Parents have been made aware. We will get the information.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized. Evidence: Child 1 does not have documentation of completed immunization records in the file. (Child 1, start date 11/26/18) Plan of Correction: The parent knows and has been reminded. We will follow up.
Standard #: 22VAC40-111-340-C Description: Based on observation, the provider did not have all the required items in the bathroom. Evidence: In the bathroom, the provider did not have paper towels, toilet paper, and liquid soup. Plan of Correction: The items were placed in the bathroom at the time of inspection.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the provider did not have the most recent inspection findings posted. Evidence: The most recent inspection on September 6, 2018 was not posted. Plan of Correction: We will post the Violation Notice going forward.
