63.2 Licensure and Registration Procedures

An unannounced renewal inspection was conducted today from 9:30am - 11:30am. There were 6 children (15 points) directly supervised by 2 staff. The home, 2 staff records, 2 children's records, 2 household members records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playong freely, dancing, singing and eating snack. Snack served today was bananas and milk. Diapering procedures were also observed. There was a supply of books, toys and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-120-B
Description: Based on record review, the provider did not ensure all staff records were complete. Evidence: Staff 1 does not have documentation of two completed references and an emergency contact.

Plan of Correction: I will get the references today and the emergency.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not ensure all children's files were complete. Evidence: Child 1 is missing one parent work phone number and a medical insurance policy number.

Plan of Correction: I will talk to the parents and get the information. The parents know I need the information.

Standard #: 22VAC40-111-200-B
Description: Based on record review, the provider did not orient staff on all required topics. Evidence: Upon interview, Staff 1 was familiar with the center and the policies but does not have documentation of orientation of orientation completed.

Plan of Correction: I Will have her sign the orientation form today.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and observation, all staff did not have a central registry finding within 30 days of employment. Evidence: Staff 1, present during inspection, does not have a completed central registry finding in their file. (Staff 1, DOH 9/4/18)

Plan of Correction: The background check was sent out on 10/22/2018.

Description: Based on record review, all staff did not submit to fingerprinting. Evidence: Staff 1 has not submitted to fingerprinting. (Staff 1, Date of Hire 9/4/18)

Plan of Correction: We will make the appointment for her as soon as possible.
