An unannounced mandated Renewal inspection was conducted today. There were 5 children and 2 staff present. (12 points) Children were observed playing house, singing songs, playing on an Ipad, watching Paw Patrol, reading, having snack. Reviewed resident files, 4 children's files, attendance, menus, fire drills, first aid supplies, and physical plant. Violations of the Standards are recorded on the Violation Notice. Description: Based on record review, provider did not obtain documentation of a physical exam within 30 days of the child's first day of attendance. Evidence: Child#3 (first day of care-9/17/18) does not have documentation of a physical exam within 30 days of their first day of attendance.

Plan of Correction: I will ask the parent for the physical

Standard #: 22VAC40-111-80-A
Description: Based on record review, provider did not obtain verification of children's identity and age within 7 days of the child's first day of care. Evidence: Child#1 (1st day- 6/5/18), #3 (1st day-9/17/18), and #5(1st day-11/3/14) did not have documentation in their file of verification of their identity and age.

Plan of Correction: I will ask parents for the birth certificates

Standard #: 63.2-1721.1-B-2
Description: Based on record review, Provider did not submit fingerprints for processing to the Department of Social Services by September 30, 2018 to as required. Evidence: Provider does not have results of fingerprint background check through the Department.

Plan of Correction: I will set up an appointment on Monday.
