Comments:
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 7 children, totaling 21 points. Children's records, caregiver records, caregiver training, background checks, emergency supplies, emergency preparedness items, children's nutrition, children's programming, and the physical plant were inspected today. The children were observed singing songs, dancing to music, listening to stories, engaging in free play activities, interacting with caregivers, and eating lunch. The children were served pork chops, baked sweet potatoes, mixed vegetables, and apple juice for lunch today. This inspection was completed between approximately 10:15am and 12:00pm. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on a review of emergency preparedness items it was determined that the first aid kit does not include all required items. Evidence: The first aid kit does not include a second triangular bandage.

Plan of Correction: I will buy another bandage this weekend.

Standard #: 22VAC40-111-920
Description: Based on observations of posted documents, it was determined that the posted menu did not include all required information. Evidence: The posted menu was not dated to reflect the current one week period.

Plan of Correction: I mark each calendar by the week. This is week 2. I will start using the actual weekly dates.

Standard #: 22VAC40-111-960-E
Description: Based on observations, it was determined that infant bottles have not been labeled as required. Evidence: The infant bottles of formula were not labeled with each child's name, and dated for the current day.

Plan of Correction: The bottles will be labeled.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: The record for assistant caregiver A does not include two reference checks.

Plan of Correction: I will complete the reference checks this weekend.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record C does not include the parent's medical insurance policy number. Child record D does not include the information for a second emergency contact person.

Plan of Correction: Both parents will be asked for the missing information.

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver training records, it was determined that one assistant caregiver has not completed all required training. Evidence: The provider has not conducted Orientation Training for assistant caregiver A.

Plan of Correction: We will do the training during nap time today.
