Thank you for your time and assistance with the announced initial inspection conducted today, December 7, 2018. The on-site inspection was held from 1:10p.m. to 4:30p.m. On this date, there were no children in the care of the provider and there were four children enrolled in the program. There were four children's records reviewed, along with the records for the provider, two assistants, and one household member. The provider will not be administering any medications at this time. The home and outside play areas were toured. There was an ample supply of toys and equipment for the children. The exceptions to compliance were recorded on the violation notice and the risk ratings for the violations were noted on the supplemental page. Some items reviewed and observed included: required posted information, manufacturing date of three cribs, rest mats and linens, refrigerator and freezer temperature, emergency supplies, first-aid kit, warm water temperature, and daily attendance record. A recommendation will be made to issue a conditional family day home license with a capacity of 12 children and an age range of 6 weeks through 12 years. Standard #: 22VAC40-111-760-A-1 Description: Based on review of the first aid kit and emergency supplies, the first aid kit did not include all of the required items. Evidence: 1. The provider's first aid kit did not include two triangular bandages or a first aid instructional manual. Plan of Correction: The provider will purchase the missing items and have them on hand as soon as possible.
Standard #: 22VAC40-111-800-A Description: Based on review of required documents and confirmation with the provider, the provider failed to have a written emergency preparedness and response plan. Evidence: 1. The provider had not completed the emergency preparedness and response plan. Plan of Correction: The provider began working on the plan during the inspection and will scan it to the inspector once it is complete.
Standard #: 22VAC40-111-320-I Description: Based on observation and discussion with the provider, the provider failed to have a fire extinguisher that met the Uniform Statewide Building Code. Evidence: 1. The fire extinguisher the provider had in the home was not an ABC portable fire extinguisher with a classification of at least a 2-A:10-B:C. The fire extinguisher present had a classification of 1-A:10-B:C. Plan of Correction: The provider will obtain a new portable fire extinguisher with the required classification over the weekend.
