Comments:
An unannounced monitoring inspection was conducted beginning at 8:40 and ending at10:00. There were 10 children in care with the provider and two assistants. The children were observed playing in the playroom with age appropriate toys and working with Tablets and dinosaurs in the classroom. Provider does not give medication. Violations:
Standard #: 22VAC40-111-50-A
Description: Based on observation, it was determined that the daily attendance sheet was not up to date. Evidence: At 8:45 a.m. , there were 10 children in care, There were 2 children's names on the attendance sheet.

Plan of Correction: The Provider will monitor the attendance sheet to assure that it is current. Names will be added as each child arrives.

Standard #: 22VAC40-111-180-A
Description: Based on a review of the records of two assistants and three family members, it was determined that 1 assistant (Assistant #1) had expired TB test. TB tests are to be redone every two years. Evidence: Assistant #1 had a TB test that was due on 8/7/18. It had not been done.

Plan of Correction: All Assistants and Family members 18 and older will have TB tests or screening every two years.

Standard #: 22VAC40-111-340-C
Description: Based on observation, it was determined that there were no paper towels in the bathroom as of 8:45 a.m.

Plan of Correction: The provider brought paper towels when it was pointed out to her. Paper Towels will always be available in the bathroom.

Standard #: 22VAC40-191-50-A-3-a
Description: Based on a review of the records of the provider, provider's family and the assistants it was determined that none of these people had done the Fingerprint Criminal Record Check. Evidence: All staff and the provider and famukly had State Police Criminal Record checks on file but none had done the Fingerprint Background checks that were due no later than 9/30/18.

Plan of Correction: The provider had misunderstood the memo regarding the checks to be paid for by DSS for another year to mean that they had another year to get them. All staff and Family members who are 18 or older will have Fingerprint tests done immediately. Provider will call the OBI office to assure that she has the Code and then appointments will be made for everyone. LI will be notified when the results are returned to the provider.
