Technical Assistance:
During the inspection, the licensing representative discussed with the provider: - meal provisions - general recordkeeping strategies - shelter in place drills to be practiced and recorded by end of the year - upcoming documentation deadlines for household member tuberculosis screenings - provider online training resources - OBI contact information

An unannounced monitoring inspection was conducted today from 10:50am-12:25pm. A total of 3 children [10 points] were present with 2 caregivers. Children were observed interacting positively with caregivers and while engaged in play with various toys and manipulatives. A sample of 3 children's records, 3 caregiver/household member records, drills and other documents were reviewed. Areas of non-compliance are documented on the violation notice. Standard #: 22VAC40-111-100-A
Description: Based on records reviewed, 2 out of 3 children's records were missing documentation of a physical examination within 30 days of enrollment. Evidence: Physical examination records were not on file for Child #2 [date of enrollment: 10/18/2018] or Child #3 [date of enrollment: 6/7/2018].

Plan of Correction: I will ask the parents to provide records by the first of the year.

Standard #: 22VAC40-111-90-C
Description: Based on a review of records, records for 1 out of 2 children under the age of 2 did not include documentation of additional immunizations every six months. Evidence: The most recent record of immunizations for Child #1 [13 months old] was obtained 2/27/2018.

Plan of Correction: I will request the information from the parent by the first of the year.

Standard #: 22VAC40-111-180-A
Description: Based on a sample review of records, 1 out of 3 caregiver/household member records, did not include documentation of subsequent tuberculosis screenings every two years from the date of the most recent screening. Evidence: The most recent record of a tuberculosis screening for Caregiver #1 was obtained 9/9/2018.

Plan of Correction: The caregiver will obtain the screening in February.

Standard #: 22VAC40-111-240-A
Description: Based on observations, areas and furnishings inside the home were not maintained in clean, safe and operable condition. Evidence: 1. The wooden frame underneath a mattress of a crib used for an infant in care was cracked - posing a concern of the stability of the mattress. 2. An area of chipped and peeling paint was observed in a hallway frequented by children in care.

Plan of Correction: I will have the areas repaired [plasted chipped paint and order/repair bottom of crib frame].

Description: Based on observations, potentially poisonous substances were not stored in an area inaccessible to children. Evidence: Several cleansers were observed in a lower cabinet in the kitchen where children have access.

Plan of Correction: Corrected during inspection; A temporary latching unit was installed on the cabinet.

Standard #: 63.2-1721.1-B-3
Description: Based on a review of records, 3 out of 3 caregiver/household member records did not include a criminal record check through the required fingerprinting vendor by the established deadline of 9/30/2018. Evidence: There were no fingerprint results on file for Caregiver #1, Caregiver #2 or Household Member #1.

Plan of Correction: Contact information to OBI was provided; I will contact OBI and schedule fingerprinting appointments.
