54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted with the provider on December 17, 2018 to determine compliance with the licensing standards. There were three day care children present, totaling 10 points. The children were waking from rest and having afternoon snack. Snack served today: hot dog and applesauce. Three children's files and one provider file was reviewed - please see Supplemental Summary for missing item. Provider does not administer medication - Medication Administration Training (MAT) is required to administer medicine to day care children. First aid and CPR certification is valid until 4/22/2019. Time of today's inspection: 2:30 p.m. to 4:00 p.m.

Standard #: 22VAC40-111-100-A Description: Based on review of three children's files, the provider failed to obtain documentation of a physical examination or well baby exam within 30 days after the first date of attendance. Evidence: Child 3 (date of enrollment 4/9/2018) did not have documentation of a physical or a well baby exam. Plan of Correction: Provider requested the physical from the parent during today's inspection.
