Discussed with provider: I cannot submit your application for approval until all required background search documents have been reviewed. The phone numbers for Office of Background Investigations (OBI) and Central Registry Searches was also given to the provider.
An unannounced renewal study was conducted today. There are 11 children present (23 points) with 2 caregivers. 4 household and 11 children's records were reviewed. See violation notice form missing information. Children were observed finishing lunch, preparing to rest, resting, during diaper changes, and while eating a snack. Emergency evacuation drills, first aid supplies, emergency supplies, rest mats and cribs are all in compliance. Please notify me immediately once you have received your background results. Evidence: 3 of 11 records reviewed were not updates annually. *Child #7 - record is dated 11/21/17 *Child #8 - record is dated 11/1/17 *Child # 11 - record is dated 11/1/17 Plan of Correction: I will have the parents update the information today.
Standard #: 22VAC40-111-60-B Description: Based on a review of records, children's records do not contain all required components. Evidence: 11 children's records were reviewed. Missing information includes but may not be limited to: *Parent's work information (employer, employer address, or phone number) (Child 10) *Proof of Birth documentation (date document was viewed, person viewing document) (Child #2) *Emergency medical authorization (Child #5) Plan of Correction: I will ask the parents to provide the missing information.
Standard #: 22VAC40-111-70-A Description: Based on a review of records, there is no written acknowledgement of receipt of all requires policies and procedures. Evidence: There is no signed Provisions of the emergency preparedness plan for Child #5. Plan of Correction: I will ask the parent to sign the form today.
Standard #: 22VAC40-111-680-A Description: Based on observation, caregiver's did not wash their hands immediately after diapering a child. Evidence: Caregiver #2 was observed changing an infant's diaper. After the caregiver was finished, she placed the child in a crib and began to diaper another child without washing her hands. Plan of Correction: I will put a sign up there to remind her to wash her hands.
Standard #: 22VAC40-111-680-B Description: Based on observation, caregivers did not ensure that children's hands were washed after a diaper change.
Evidence: Caregiver #2 was observed changing a child's diaper. When the caregiver was finished, the child was placed into a crib without washing their hands.
Plan of Correction: I will remind her wash the children's hands.
Standard #: 22VAC40-191-40-D-5-a Description: Based on a review of records, one Household member does not have a completed Central Registry Search on file with in 30 days of turning 14 years old. Evidence: Household member (HHM) #1 does not have a completed Central Registry search on file. (Date of Birth : 03/12/2004) Plan of Correction: The search was mailed a the beginning of this month. I will contact Richmond to make sure they received the form.
Standard #: 63.2-1721.1-B-2 Description: Based on a review of records, adult household members have submitted to a nationwide fingerprint search prior to September 30, 2018. Evidence: No Household Members have had a fingerprint search conducted. (HHMs #2, 3, 4) Plan of Correction: I contacted OBI during the inspection to get the codes for fingerprinting. We will schedule appointments as soon as possible.
