An unannounced renewal inspection was conducted today from 10:30am-1:00pm. There were 12 children (29 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, doing worksheets, practicing numbers and shapes, singing, washing hands and eating lunch. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-830-D
Description: Based on record review and interview with the provider, documentation of evacuation drills was found to be missing or incomplete. Evidence: Documentation of required monthly evacuation drills were not on file for September 2018, only a shelter-in-place drill was conducted in September.

Plan of Correction: We did the drill but did not document it. We will make sure that we document all drills, we will conduct two drills this month.

Standard #: 22VAC40-111-110-B
Description: Based on record review, the family day home did not have physical examination documentation that was signed or stamped by a doctor, his designee, or local health department official. Evidence: The records of Child#1 and Child#5 contain a physical form that was not completed by a physician, designee, or an official of a local health department.

Plan of Correction: We will have the parents take the forms back to their pediatrician.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were not maintained. Evidence: Child#1's record is missing the mother's work address. Child#3's record is missing the parent's employer's name, their work address and the father's work telephone number. Child#4's record is missing the father's work address. Child#5's record is missing the parent's work telephone number, the father's work address, the address of both emergency contacts, and the child's medical insurance policy number.

Plan of Correction: We will request the information today.

Standard #: 22VAC40-111-70-A
Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance. Evidence: Child#5's record is missing the family day home's information for parents form, and the Liability Insurance Declaration form.

Plan of Correction: We will obtain a copy from them.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the family day home did not obtain documentation of additional immunizations once every six months for children under the age of two years.

The last immunization record on file for Child#4 was dated 11/09/2015, the child was 15 months old when the last immunization record was obtained.

Plan of Correction: We recently asked for it, they will bring it to us as soon as they have it.

Standard #: 63.2-1721.1-B-2
Description: Based on record review and interview, the provider did not obtain a fingerprint based background check for a household member. Evidence: Household#3 was required to have a fingerprint background search completed by 09/30/2018. The fingerprint based background check was not conducted.

Plan of Correction: We will do it as soon as possible and submit the form to DSS.
