Comments:
An announced initial inspection was conducted today from 10:20am-1:25pm. One child [2 points] was present with two caregivers. Lunch and handwashing were observed, along with nurturing interactions and positive redirection. 2 caregiver records, 6 household member records, 2 children's records, emergency drills, emergency supplies, physical plant, outdoor play areas, and other documents were reviewed/inspected. Description: Based on observations, outside of the home were not maintained in clean, safe, and operable conditions. Evidence: Areas of rust and hard, cracked plastic were observed on the chains of 2 swings on an outdoor swing set. The seats of the swings were broken with exposed rusting screws.

Evidence: A protective barrier or guardrails on both sides were not installed on stairs with eight risers leading from the basement to the first floor from a primary programming area, while a 2 year old child was in care.

Description: Based on records reviewed, an out of state central registry was not obtained for a household member that has resided outside of the state of Virginia within the preceding five years. Evidence: Household member #6 disclosed on a sworn statement signed on 8/21/2018 that they have resided in Pennsylvania within the past five years.
