Technical Assistance:
Technical assistance was provided on the following: Staff training hours, children's records, MAT forms.

Comments:
An unannounced monitoring inspection was conducted from 10:45am to 12:00pm. There were 9 children (23 points) in care of two caregivers. The children were observed playing in groups with legos, toys, listening to music, washing hands and eating lunch. The lunch menu included salmon, broccoli, strawberries, pasta and milk. 5 children's and 2 staff records were reviewed. Areas of non compliance are listed in the violation notice. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on observation and staff interview, the provider did not maintain an up to date record for each child enrolled. Evidence: 1. Staff # 1 stated that Child # 1's first date of enrollment was 1/2/19. 2. A complete record for Child # 1 was not maintained including information such as emergency contact information, medical insurance information, liability insurance and medication policy forms, information for parents and parent agreement.

Plan of Correction: I will have the parent complete the registration form and bring in all required information.

Standard #: 22VAC40-111-90-A
Description: Based on observation, the family day home did not obtain documentation of immunizations for a child enrolled before the first day of enrollment. Evidence: 1. Child # 1's (7 months old) first day of enrollment was 1/2/19. 2. The family day home provider did not obtain immunization records for Child # 1 before the first day of enrollment. There were no immunization records available for review.

Plan of Correction: Child's parent will bring in the immunization records.
