Discussed creating and implementing systems to maintain compliance with child, caregiver, and household member records. Provided guidance on background checks.
An unannounced renewal inspection was conducted today from 9:45am-1:00pm. There were 6 children (20 points) supervised by 2 direct care staff. The home, 2 caregiver records, 8 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely, having storytime, listening to music, and eating lunch. Lunch today was chicken, bread, fruit, and milk. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-A Description: Based on record review, the provider did not maintain an up-to-date record for 1 of 6 children enrolled. Evidence: Child #4 (start date: 1/2/2019) did not have documentation of an up-to-date record. Plan of Correction: We will get the child's record from parent.
Standard #: 22VAC40-111-60-B Description: Based on record review, 4 of 8 children's records did not contain all of the required information. Evidence: 1. Child #2's (start date: 9/25/2015) record did not contain documentation of a physical form completed by a physician. 2. Child #3's (start date: 6/20/2016) record did not contain documentation of parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70. 3. Child #6 and Child #7's (start dates: 7/24/2017) records did not contain documentation of physical forms completed by a physician, and parent signed acknowledgement of the medication policy. Plan of Correction: We will get the missing information from the parents.
Standard #: 22VAC40-111-170-A Description: Based on record review, 2 of 4 caregiver and household member records did not contain documentation of current reports of TB screening. Evidence: 1. Caregiver A and Household Member B's records did not contain documentation of current reports of TB screening forms. 2. Caregiver A's record contained documentation of TB test completed on 12/19/2018. The individual is awaiting the results in the mail. Plan of Correction: We will get the missing TB forms.

Household member A's record did not contain documentation of central registry findings and a completed sworn disclosure statement. 2. Household member B's record did not contain documentation of central registry findings. Plan of Correction: We will have them by the end of the month.
