54.1-3408 Provider must be MAT certified to administer prescription medication.

Comments:
An unannounced renewal visit was conducted today between the hours of 9:55am through 11:45am with the provider on site. There were 9 children (29 points) with the provider and two assistants. A sample of 4 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, reading books, singing and Spanish learning activities. . Interactions between the children and staff were positive. The physical space, emergency supplies, evacuation drills, attendance records and staff training were reviewed. Standard #: 22VAC40-111-120-B
Description: Based on record review, the assistants record did not contain all required documents. Evidence: 1. Assistant #2 was hired on 12/10/2018 and did not have two references in the file.

Plan of Correction: I will request two references from her.

Standard #: 22VAC40-111-70-B
Description: Based on record review, the provider did not obtain the parent's written acknowledgement of required information before the child's first day of attendance. Evidence: 1. Child #1 (start date 5/17/2018) and Child #4 (start date 4/03/2018) did not have the parent's written acknowledgement of receipt of the parent information form

Plan of Correction: I will have the parent's review and sign the forms.
