Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:45am - 11:45pm. There were four children in care for a total of thirteen points with two caregivers. The children were observed playing with the available toys and watching cartoons. Five children's records were reviewed. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-100-A
Description: Based on record reviews, the family day home failed to obtain documentation of a physical examination. Evidence: 1. The inspector reviewed five children's records. Two of five records reviewed (child record #2 and #5) did not contain documentation of a physical exam. Child record #2's date of enrollment was 12/01/18 and child record #5's date of enrollment was 12/01/18.

Plan of Correction: The provider will request this information from the children's parents again today.

Standard #: 22VAC40-111-60-B
Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. The following information was missing from the children's records: Child record #1 did not contain an address for the children's doctor and there were no addresses for the emergency contacts. Child record #2 did not contain proof of child's identity and age and child's date of enrollment was 12/01/18. Child record #3 did not contain addresses for the emergency contacts. Child record #4 did not contain any information for emergency contacts. Child record #5 did not contain proof of the child's identity and age and child's date of enrollment was 12/01/18.

Plan of Correction: The provider will request this information again from the children's parents today.

Description: Based on record reviews, the family day home failed to obtain signed acknowledgment of liability insurance declaration. Evidence: 1. The inspector reviewed five children's records. Two of five records reviewed (child records #3 and #4) did not contain signed liability insurance declarations.

Plan of Correction: The provider will request this information again from the children's parents today.

Description: Based on record reviews, the family day home failed to obtain documentation of immunizations. Evidence: 1. The inspector reviewed five children's records. Two of five records reviewed (child record #1 and #4) did not contain documentation of immunizations.
