- Technical Assistance:
Consultation provided on assistants/caregiver requirements and the level of supervision required for those assistants between 16 and 17 years of age.
An unannounced monitoring inspection was conducted today with the provider who was present with 6 children ( between the ages of 4 months and 10 years) for a total of 8 points. For lunch the children were served spaghetti with meat sauce, caesar salad, strawberries and milk. One injury/accident report was reviewed and there were no medications on site for any of the children. Six children and three household member's files were reviewed as well. Arrival time: 10:10 am Departure time: 11:55 am
Standard #: 22VAC40-111-100-A Description: Based on a review of six children's files, it was determined that two children did not have physicals on file within 30 days of enrollment. Evidence: Child A and B did not have physicals on file. Plan of Correction: I will get the physicals.
Standard #: 22VAC40-111-60-A Description: Based on a review of six children's files and an interview with the provider, it was determined that children records were not complete. Evidence: There was no second emergency contact identified for child A and B and there was no phone number for one of the emergency contacts for child C. There was no enrollment date for child D and no employment information on one of the parents for child C. Child A enrolled on 7/2/18, child B enrolled on 6/11/18 and child C enrolled on 5/9/16. Plan of Correction: I will review all the files and make sure they are complete.
Standard #: 22VAC40-111-150-A Description: Based on observation and interviews, it was determined that an 11 year old was functioning as an assistant when the age requirements for assistants are16 years of age. Evidence: The Licensing Inspector observed in the upstairs bathroom an 11 year old household member changing and caring for a 2 year old who had soiled their pants. The provider was downstairs in the basement with the other 5 day care children. The 11 year old was functioning as an assistant. Plan of Correction: I will make sure any assistant is at least 16 years of age and always under my direct supervision if they are under 18 years of age.
Standard #: 22VAC40-111-170-A Description: Based on an interview and a review of household member's files, it was determined that one household member did not have a tuberculosis test/screening on file. Evidence: Person A did not have a tuberculosis test/screening on file. Plan of Correction: We will get the TB.
Standard #: 22VAC40-111-240-A Description: Based on observation , it was determined that the family day home was not maintained in a safe manner. Evidence: The Licensing Inspector observed a container of bleach on the counter in the kitchen accessible to the children. Plan of Correction: The bleach was removed.
