Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and training hours obtained in 2018. As a reminder, annual training hours, are hours obtained in addition to the satisfactory completion of first aid training and CPR training.

The following inspection took place between the hours of 10 am through 12:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Lovettsville, Virginia. At the start of this unannounced monitoring inspection there were a total of 7 children (16 points) with 3 caregivers. A total of 6 children?s files, 4 staff files, and 4 household member?s files were reviewed. The children were engaged in the following: breakfast, toy play, song, learning, diapering, hand washing, drop off, pick up, and other organized activities. Description: Based on review and interview, 1 of 4 household member's files reviewed indicated that the Provider did not obtain for each adult household member (prior to coming into contact with children) a current Report of Tuberculosis (TB) Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form. Evidence - 1. On the date of inspection (01/04/2019), current TB documentation for Household Member #2 was not available for review. 2. The Provider stated (during an interview conducted on the date of inspection) that a TB test was administered, but never read.

Plan of Correction: I future test to make up for this missed reading will be scheduled.
