An unannounced monitoring visit was conducted today between the hours of 10:00am through 12:00pm with the provider on site. There were 2 children (5 points) with the provider. A sample of 4 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, being fed a bottle and napping. Interactions between the children and provider were positive. The physical space, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-111-830-B Description: Based on record review, the provider failed to practice shelter-in-place procedures a minimum of twice per year. Evidence: 1. The provider did not record that two shelter in place drills had been completed in 2018. Plan of Correction: I will make sure we practice and record the drills.
Standard #: 22VAC40-111-70-A Description: Based on record review, the provider did not provide the parent's in writing all required informtion. Evidence: 1. Child #1 did not have a parent's signature or form regarding whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia. Plan of Correction: I will have the parent review and sign.
Standard #: 22VAC40-111-210-A Description: Based on record review, the caregivers did not obtain 16 hours of training annually. Evidence: 1. The provider had 13 hours of training in 2018. 2. The Assistant did not complete any training during the period 9/01/2017 thru today. Plan of Correction: I will make sure we complete 16 hours of training.
