Thank you for your time and assistance with the unannounced monitoring inspection conducted today, January 9, 2019. The on-site inspection was conducted from 2:20p.m. to 5:35p.m. On this date, there were 11 children in the care of the provider and two assistants for 32 points. There were a total of 22 children enrolled in the program. There was five children's records reviewed. The records of the provider, a household member and two assistants were reviewed. Three caregivers were interviewed. There were no medications being administered at the time of the inspection, which is the policy of the provider. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. There were an ample supply of age appropriate toys and equipment. Some items reviewed and observed were: nap time, infant care, afternoon snack, diaper changing procedures, free play, outdoor play, emergency evacuation drills, required posted information, and daily attendance record. Standard #: 22VAC40-111-60-B Description: Based on review of five children's records and discussion with the provider, the records of two children did not contain documentation that the parents had reviewed the children's emergency contact information after the first year of attendance. Evidence: 1. The record of Child #1 did not contain documentation that the parent had reviewed the emergency contact information. Date of enrollment for Child #1 was 09/19/2017. 2. The record of Child #2 did not contain documentation that the parent had reviewed the emergency contact information. Date of enrollment for Child #2 was 07/13/2017. Plan of Correction: The provider will have the parents of the children review the emergency contact information for any changes as soon as possible.
