54.1-3408 Provider must be MAT certified to administer prescription medication.

An initial Licensing Inspection was conducted beginning at 9:30 . and ending at 11:50, There were 2 infants in care with the provider and an assistant. The children were observed having a snack and playing with age appropriate toys. The home is well kept and the day care area takes the entire lower level with several rooms and two bathrooms, a small kitchen. Supplies exceed requirements. Reviewed family and staff files - all complete including background checks, TB tests . Reviewed children's files - See violation notice. Emergency preparedness plan is complete. Description: Based on observation, it was determined that the first aid kit is not complete. Evidence: The first aid kit does not have two triangular bandages.

Plan of Correction: The provider will add two large scarves to the first aid kit.

Standard #: 22VAC40-111-770
Description: Based on observation and interview with the provider, it was determined that the radio that is with Emergency supplies is not a Weather Band Radio as required by the Standards. Evidence: The radio is a regular battery operated radio.

Plan of Correction: The provider will get a Weather Band Radio,

Standard #: 22VAC40-111-60-B
Description: Based on a review of the records of 4 enrolled children, it was determined that not all records were complete. Evidence: The file for Child #1 is missing the medical insurance policy number and the Well Baby Check; The file for Child #2 is missing a second emergency contact with complete addresses and a Well Baby Check; The file for Child #3 is missing work phone number for parents and addresses for emergency contacts; The file for Child #4 is missing Drs phone number, emergency contact addresses and well baby check.

Plan of Correction: The provider will notify parents today of the things that are missing and will give them a time frame to have everything in place.

Description: Based on a review of the record for the Assistant, it was determined that her file is not complete. Evidence: The file for Caregiver A does not have documentation of the Assistant's Orientation training.

Plan of Correction: The provider will complete the Orientation this week,

Standard #: 22VAC40-111-430-B
Description: Based on observation, it was determined that the gate at the bottom of the stairs in the daycare area is not securely fastened to the opening. Evidence The gate that is at the stairs is pressure mounted.

Plan of Correction: The provider will get a gate that can be screwed into the wall .
