An announced initial inspection was completed on January 7, 2019 from approximately 10:00a.m. to 12:35p.m. One caregiver's, one household member's record, the emergency preparedness and response plan and required postings were reviewed. The physical environment, play equipment and materials, nutrition, transportation and the home?s first aid and emergency supplies were inspected. The provider is planning for a capacity of twelve, will provide transportation when needed, has liability insurance and will administer medication. The provider was available for interview, the inspection and the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the provider. There was one citation for a violation of the Standards for Licensed Family Day Homes. See the violation notice on the Department's public web site for violations of the Standards.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the outdoor space, the licensee failed to ensure that areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence: 1. The back deck, used by children in care for outside play time, was observed to have protruding nails, in approximately four places on the deck boards and railings.

Plan of Correction: The provider will hammer the nails down flush with the decking and railing boards.
