Discussion was held on the topics of current enrollment, current staffing, and the current expiration dates for background checks; all obtained background checks are good for 5 years from the date of that check.
The following inspection took place between the hours of 12 pm through 2:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Aldie, Virginia. At the start of this unannounced monitoring inspection there were a total of 10 children (30 points) with 2 caregivers. A total of 10 children?s files, 2 staff files, and 1 household member?s file was reviewed. The children were engaged in the following: nap time, afternoon snack, toy play, center play around the room, diapering, bathroom break, hand washing, and other organized activities. Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 10 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (01/15/2019), documentation of a physical was not available for review for Child #10. 2. The start date listed for Child #10 was 12/08/2016. 3. Child #10 was in attendance on the date of inspection. Plan of Correction: The Parent of Child #10 shall be contacted and documentation of a physical shall be obtained.
Standard #: 22VAC40-111-60-B Description: Based on review, 1 of 10 children's files reviewed indicated that not all of the required information/documentation was contained within each child's record. Evidence - 1. On the date of inspection (01/15/2019), the information of 1 of 2 emergency contacts was listed. 2. While there was two spaces to complete, the second space for the next emergency contact was left blank. Plan of Correction: The Parent of Child #9 shall be contacted and this information shall be placed in the child's record.
