Comments:
An unannounced full renewal inspection was conducted today. The provider and two assistant caregivers were caring for 12 children, totaling 34 points. Children's records, background checks, caregiver records, caregiver training, administrative documents, the physical plant, children's programming, and children's nutrition were inspected today. The children were observed engaging in circle time discussions, singing songs, playing with toys, eating lunch and taking their afternoon nap. The children were served egg salad sandwiches, peas, pineapple, and milk for lunch today. This inspection was completed between approximately 10:15am and 12:20pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of children's health records, it was determined that one record was not complete. Evidence: Child record E does not include a physical exam report. This child has been in attendance for more than 30 days.

Plan of Correction: The parent gave me the wrong form. I will ask for the correct form.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that two records were not complete. Evidence: Assistant record A does not include: verification of age; emergency contact information; documentation of two reference checks. Assistant record B does not include: verification of age; emergency contact information; documentation of two reference checks.

Plan of Correction: The emergency contact information was documented immediately during the inspection. The other other information will be gathered this week.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record C does not include the parent's medical insurance policy ID number.

Plan of Correction: I will ask the parent for the missing information.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's immunization records, it was determined that one record is not complete. Evidence: Child record E does not include an immunization record.

Plan of Correction: The parent gave me the wrong form. I will ask for the correct form.

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver training records, it was determined that there is no documentation of one caregiver's required orientation training. Evidence: There is no documentation of completed Orientation Training for caregiver A.

Plan of Correction: I have misplaced the training documentation. I will re-create the training and update the documentation in the file.
