An unannounced renewal inspection was completed today. The physical plant, 4 caregiver/household member files, 5 children's files, caregiver training, children's activities, emergency procedures, drills and supplies were inspected between 9:40-10:50. There were 5 children, totaling 11 points, under the supervision of the Provider. Today, the children watched a video, ate morning snack, and enjoyed free play. The children bring their lunches and snacks to the FDH each day that they are in care. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs. Child #1 is 16 months old. The most recent immunization record was completed when the child was 6 months old. Plan of Correction: The Provider will ask the parents for this.
Standard #: 22VAC40-111-170-A Description: Based on interview, 1 household member does not have a completed TB test/screening. Evidence 1. Household member #43 does not have a TB test/screening. Plan of Correction: This will be completed.
Standard #: 22VAC40-111-280 Description: Based on observations, there was a deodorizer stored in an area accessible to children. Evidence 1. In the cabinet under the sink in the bathroom, there was a bottle of Febreeze spray. Plan of Correction: This will be moved.
Standard #: 22VAC40-191-40-D-4-a Description: Based on interview, 1 adult household member does not have all required background checks. Evidence 1. Household member #3 does not have a search of the central registry (SCR) completed after turning 18 years old. Plan of Correction: The SCR will be sent out this week.

Evidence 1. Household member #4 does not have a completed SCR. Plan of Correction: The SCR will be sent out this week.
