Technical Assistance:
Make sure to vary the time of day you do escape and shelter in place drills. Escape drills are to be conducted monthly while shelter in place drills are to be conducted twice a year. Consultation provided on sworn disclosure forms and all background checks are to be renewed every five years.

An unannounced renewal study was conducted today with the provider, who was present with an assistant and six children for a total of 15 points. Five of the children were engaged in free play before lunch while one child was napping in their crib. No child had medication on site and the provider reported there have been no injuries or accidents. Six children, 3 household members and one assistant's files were reviewed. The provider's First Aid/CPR are through American Safety Health Institute and is due to expire in March 2020. Arrival time: 11:30 am Departure time: 1 pm

Description: Based on a review of six children's files and an interview with the provider, it was determined that child A ( enrolled on 11/26/18) did not have the phone number listed for one emergency contact and did not have the address listed for the second emergency contact.

Plan of Correction: I got the information during the inspection today.

Standard #: 22VAC40-111-460-D
Description: Based on a review of the two household pets vaccination records and an interview with the provider, it was determined that Bruno's rabies vaccination had expired on 11/15/16 and Buddy's rabies vaccination had expired on 12/23/16.

Plan of Correction: We will up to date both pets rabies vaccinations.
