Comments:
An unannounced renewal inspection was conducted on Wednesday, January 23, 2019 from approximately 10:00 AM to 11:45 AM to determine the provider's compliance with licensing standards. In attendance were three non-resident children. The children were being supervised by one caregiver during the inspection. There was a total of six points based on the children?s ages. The children were observed watching children?s programs on television, playing with toys, singing songs and sleeping. Lunch consisted of cheeseburgers, broccoli, strawberries and milk. A walk through of the home was conducted with the provider. The provider stated the outdoor playground area has not been used due to recent weather conditions.

During the inspection, the licensing inspector reviewed the following: emergency supplies including first aid kit, battery operated weather radio, emergency plan, emergency drill logs, required postings, menu, caregivers? training hours and daily attendance. The provider stated medications are not being administered at this time. Transportation is not provided. During the inspection four children?s and two caregivers? records were reviewed.

Violations:
Standard #: 22VAC40-111-30-A
Description: Based on interviews and documentation review, the provider did not ensure compliance with the terms of the current license issued by the department.

Evidence:
During the inspection, the licensing inspector reviewed the record for Child #3 (start date 07/23/18). The information in the record indicated Child #3 is eight months old. Caregiver #1 stated Child #3 attends the family day home two days a week. The current license issued by the department is for children one year through 12 years, 11 months of age.

Plan of Correction: I will not provide services to Child #3.

Standard #: 22VAC40-111-50-A
Description: Based on record review and interview, the provider did not maintain a written record for children in attendance each day.

Evidence:
During the inspection, the licensing inspector asked the provider for the daily attendance for the week. The provider stated she could not locate the written attendance for the week. There were three children in care during the inspection.

Plan of Correction: I had completed the attendance for the week and I could not locate it during the inspection. I will make sure I keep the attendance in a place I can locate in the future.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure all unused electrical outlets and surge protectors contained protective covers. Evidence: During the inspection, the licensing inspector observed two outlets in the kitchen and a surge protector and outlet in the living room that did not contain protective covers.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-320-E
Description: Based on observations, the provider did not ensure that portable electric heaters located in areas accessible to children had barriers or screens.

Evidence:
During the inspection, the licensing inspector observed a portable heater in use, located in the kitchen. The heater did not have a barrier or screen around it. Children were observed in the kitchen, near the portable heater during the inspection.

Plan of Correction: I will move the portable heater to a location where it will have a barrier or screen.

Standard #: 22VAC40-111-520-A
Description: Based on observation and interview, the provider did not ensure a crib was provided for a child from birth through 12 months of age to sleep in.

Evidence:
During the inspection, the licensing inspector observed the record for Child #3 (start date 07/23/18) indicated the child is eight months of age. The provider stated that Child #3 attends the family day home two days a week. The licensing inspector asked the provider if Child #3 had a crib to sleep in. The provider stated Child #3 has been sleeping in a "pack-n-play".

Plan of Correction: I will not provide services to Child #3.
