A Subsidy Health and Safety inspection was conducted on 1/29/19 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 1/29/19. There were 10 children present from 18 months through five years with the provider and assistant supervising. The point total was 20. Five children's records (one newly enrolled child), the provider's record, and three assistant's records (one newly hired) were reviewed. No medications are administered by the provider. The inspector arrived at the provider's home at 7:45 am and departed at 12:00 pm. The children were observed at arrival, during free play time, and during lunch. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on observation, the provider failed to ensure that a written record of daily attendance documenting arrivals as it occurred was maintained. Evidence: 1. The provider was asked for the attendance at approximately 10:00 am. Three children were observed arriving between 8:30-9:30 am. These children had not been marked present on the attendance as they arrived.

Plan of Correction: This was corrected during the inspection. The provider marked all children present on the attendance when the inspector asked for the attendance.
