63.2 Licensure and Registration Procedures
An unannounced monitoring inspection was conducted with the provider. There were five children present ranging in age from 1 year to 3 years, totaling 12 points. The children were playing with blocks and watching an age appropriate tv show. Five children's files and two household files were reviewed - please see Supplemental Summary for missing items. The provider does not administer medication - Medication Administration Training is required to administer medicine. The last fire place inspection was conducted 8/17/2018. The pet vaccinations are current - two will be updated on 2/16/2019. Evacuation drills have been conducted monthly and the last shelter in place drill was conducted 10/15/2018. Standard #: 63.2-1721.1-B-3 Description: Based on review of two household files, the provider failed to have documentation of an out of state search of the central registry (CPS check) in any other state in which the individual resided in the preceding five years. Evidence: there was no documentation of a search of the central registry for the state of New York, where Household member 1 resided in the previous five years. Plan of Correction: Licensing inspector will send the CPS form to the provider who will complete and submit the form to the state.
