22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was completed on January 23, 2019 from approximately 1:00p.m. to 3:30p.m. Four children were in attendance, all four years old, for a total of eight points. The children were observed interacting with the provider and a substitute caregiver/assistant, during toileting, hand washing, lunch time, during a group coloring activity and while watching television. The physical environment and equipment and the home?s first aid kit and emergency supplies were inspected. Five children?s records, two caregiver records, one household member's record, the emergency preparedness and response plan, fire drill, shelter in place and lock down drill logs, daily attendance and required postings were reviewed. Transportation is not provided, medication is not administered and there were no injury reports to review. The provider was available for interview, the inspection and exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the provider. There was one citation for a violation of the Standards for Licensed Family Day Homes. See the violation notice on the Department's public web site for violations of the Standards.
Standard #: 22VAC40-111-780-B Description: Based on a review of children?s records and interview, the licensee failed to review with the parent annually, the child?s emergency contact information to ensure the information is current for one or five children. Evidence: 1. Child #5's record, enrolled July 15, 2017, lacked documentation of an annual emergency contact review. 2. In an interview, Caregiver #1 advised Child #5 has not been in care consistently since this summer and is more of a "drop in", but acknowledged a review was not completed. Plan of Correction: The provider will contact the parent to determine if she wishes for the child to remain enrolled. The provider will then update the emergency contacts if necessary.
