Technical Assistance:
Make sure you are doing annual review of your emergency prepardeness plan and documenting the date of the review. When the forms asks the amount of time it took to conduct the escape drill it just wants the amount of time it took you and the children to exit the house. Make sure you are updating the information on the child's record annually to include parent address, their work info, physician information, emergency contact information etc. Subsequent tuberculosis tests/screenings must be conducted every two years. Please make sure you are using the new sworn disclosure forms which you can find on the VDSS public web site.

An unannounced renewal study was conducted today with the provider, who was present with five children for a total of 10 points. The children were in the process of awakening at the beginning of the inspection. The children washed their hands and then were given snack which consisted of rice crispy snacks, banana and milk/water. No child had medication on site. Four household members files were reviewed as well as 5 children's files. Arrival time: 2:30 pm Departure time: 4 pm

Description: Based on a review of the attendance log and an interview with the provider, it was determined that the provider is not maintaining a written record of children in attendance each day. Evidence: Five children were present in the family day home however there was no written record of the children being present.

Plan of Correction: I will make sure to keep an up to date attendance.
