63.2 Licensure and Registration Procedures

An unannounced monitoring visit was conducted today between the hours of 8:55am through 11:15am with the provider on site. There were 2 children (6 points) with the provider. A sample of 3 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, nap time, bottle feeding and listening to children's music. Interactions between the children and provider were positive. The physical space, emergency supplies, evacuation drills and attendance records were reviewed. Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider failed to review with the parent the emergency contact information to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. Evidence: 1. Child #1 began with the provider on 4/24/2017 and did not have the parent's signature of a review of emergency contact information.

Plan of Correction: I will ask the parents to review, update and sign it.

Standard #: 22VAC40-111-800-A
Description: Based on interview, the provider was not able to locate a written emergency preparedness and response plan. Evidence: 1. The provider stated she could not locate the written emergency preparedness and response plan.

Plan of Correction: I will look for it or write up a new one.

Standard #: 22VAC40-111-70-B
Description: Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of the information regarding the liability insurance declaration. Evidence: 1. Child #2 (began 2/04/2019) and Child #3 (began 1/25/2019) did not have a parent's signature of the liability insurance declaration form in their file.

Plan of Correction: I will ask the parents to review and sign.

Standard #: 22VAC40-111-170-A
Description: Based on record review, the provider did not obtain from each adult household member a current Report of Tuberculosis Screening form documenting the absence of tuberculosis. 1. Household Member #1 did not have a Tuberculosis Screening form in the file documenting the absence of tuberculosis.

Plan of Correction: I will schedule an appointment for the household member.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface did not meet all requirements. Evidence: 1. The diapering surface had three small holes on the diaper pad and was not considered nonabsorbent or washable.

Plan of Correction: I will buy a new one.
