Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the status of a background check. Provider contacted OBI and was informed that the remaining background check should be mailed to the home by the end of the week. Once obtained please contact the Division of Licensing for its review. Family Day Home (FDH) license will not be issued until this review has been conducted.

The following inspection took place between the hours of 9:45 am through 1 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Alexandria, Virginia. At the start of this announced initial inspection there were a total of 5 children (1 resident child) (12 points) with 1 caregiver. A total of 5 children?s files, 1 staff file, and 4 household member?s files were reviewed. The children were engaged in the following: play, games, learning, diapering, hand washing, lunch, infant feeding, nap time, and other organized activities. Description: Based on review, a working portable battery-operated weather band radio was not available. Evidence - 1. On the date of inspection (01/30/2019), the Family Day Home (FDH) did not have a working portable battery-operated weather band radio. 2. The FDH had an AM/FM radio on site.

Plan of Correction: A working portable battery-operated weather band radio shall be purchased for the FDH.

Description: Based on review, 1 of 5 children's files reviewed indicated that the documentation of the child's emergency contact information required in 22 VAC 40-111-60 B 2 did not include the parent's initial signed acknowledgment. Evidence - 1. On the date of inspection (01/30/2019), the signature for the Parent of Child #5 was not available. 2. The start date for Child #5 was listed as 01/21/2019.

Plan of Correction: This signature shall be obtained.

Description: Based on review, 3 of 5 children's files reviewed indicated that not all of the required information was contained within each file. Evidence - 1. On the date of inspection (01/30/2019), the complete address for both emergency contacts for Child #2 was not available for review. 2. The policy number for the listed medical insurance for Child #3, and Child #4 was not available for review.

Plan of Correction: All missing information shall be obtained and placed in the applicable child's file.
