Standard #: 22VAC40-111-240-A Complaint related: No Description: Based on observation, areas of the family day home were not maintained in a clean condition. Evidence: The wall located behind the crib closest to the door of the second bedroom was stained with speckled yellow marks approximately the size of a dime. Plan of Correction: I will clean it with a wipe.
Standard #: 22VAC40-111-480-F Complaint related: Yes Description: Based on interview with provider, toys mouthed by children were not cleaned and sanitized daily. Evidence: The provider stated that she was given a stuffed toy for Child B at the time of enrollment (02/05/2018), that Child B would chew on this stuffed toy to go to sleep everyday. When the child was dis-enrolled (11/07/2018), the provider stated that she returned the toy to the parent in a plastic bag and suggested that the parent wash the toy. The provider stated "the toy smelled really bad". When asked, the provider stated she did not wash this toy while the child was enrolled. Plan of Correction: It was a unique situation because we normally don't receive toys from home. In the future I will not accept toys brought from home. I sanitize my toys right after children play with it especially if the toy is in their mouths.
Standard #: 22VAC40-111-530-D Complaint related: Yes Description: Based on observation, linens were not cleaned when soiled. Evidence: The linens in the crib located near the window in the second bedroom were soiled with a bodily fluid mark approximately the size of two nickels. At the time Child A was sleep in the crib. Plan of Correction: I washed the linens the Friday after the first visit.
