Technical Assistance:
Technical assistance today included record keeping and physical environment. The number for the fingerprint results is 800-- 799-1067.

An unannounced monitoring inspection took place today from 1:20 -3:45 pm. There were 12 children present with the provider and three staff for 35 points. I observed the end of naptime and play time. Children were playing with toys, looking at books, and learning about animal sounds and singing Old McDonald and If Your Happy And You Know It.... Records were well-organized. Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records today, not all child had documentation of a physical exam with 30 days of beginning care. Evidence: Child #1 had documentation of shots but no physical exam available today.

Plan of Correction: I will talk to the parent.

Standard #: 22VAC40-111-180-A
Description: Based on a review of records today, a household member did not have a current TB screening as required. Evidence: The TB screening for Household Member # 2 expired 9/18.

Plan of Correction: He will get a new one.

Standard #: 22VAC40-111-240-A
Description: Based on observation during today's inspection, there were some areas not maintained in a safe condition. Evidence: In the reading area today, a plastic hook was broken and had sharp edge at child height, and there was a bobby pin in the fabric above the book area. There was also a long ribbon that was accessible.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-191-50-A-1-a
Description: Based on today's inspection of staff records, it was found that staff did not have the current form for use for the Sworn Disclosure Statement. Evidence: The provider and Staff # 1, 2, 3 completed the outdated Sworn Statement.

Plan of Correction: We will complete the new form today.

Standard #: 22VAC40-191-60-G-1
Description: Based on a review of staff and household records today, fingerprint results were not available today for review.. Evidence: Although the provider and three staff had documentation of appointments to get fingerprints from August - November 2018, there was no documentation of results.

Plan of Correction: I will call the Fieldprint Office for the results.

Standard #: 63.2-1721.1-B-2
Description: Based on a review of records, fingerprinting had not been conducted for one Household Member. Evidence: Although the older Criminal Record Check was completed, it no longer meets the requirement for fingerprinting.

Plan of Correction: He will get the fingerprint.
