Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, February 6, 2019. The on-site inspection was from 10:55 a.m. to 1:30 p.m. On this date, at the time of arrival, there were four children and one resident child in the care of the provider (11 points). There were a total of four children enrolled in the program. The exceptions to compliance were noted on the violation notice and the risk ratings were noted on the supplemental page. The records of four children were reviewed. There was no medication being administered, which is the policy of the provider. The records for the provider and two household members were reviewed for updates. Lunch was provided by the parents. Some items reviewed/observed included: required posted information, water temperature, temperature of the fridge and freezer, outdoor play, bathroom and hand washing procedures, diaper changing procedures, lunch, free play with toys and age appropriate equipment, story time, and nap time. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on discussion with the provider, the provider failed to practice monthly emergency evacuation procedures. Evidence: 1. The provider stated she had not conducted any emergency evacuation drills since becoming licensed on 12/07/2018.

Plan of Correction: I will complete evacuation drills for the ones I missed in December and January and then do one each month from February on.

Standard #: 22VAC40-111-100-A
Description: Based on review of four children's records and confirmation from the provider, there was no physical examination report for one child. Evidence: 1. The record for Child #1 did not contain documentation that the child had received a physical examination from a physician. Date of enrollment for Child #1 was 12/07/2018. 2. The provider confirmed she had not obtained Child A's physical exam report.

Plan of Correction: The provider will talk to the parent of child today at pick-up time and ensure a physical examination report is obtained as soon as possible.

Standard #: 22VAC40-111-90-A
Description: Based on review of four children's records and verification from the provider, there was no immunization record for one child. Evidence: 1. The record for Child #1 did not contain documentation that the child had been adequately immunized for her age. Date of enrollment for Child #1 was 12/07/2018. 2. The provider confirmed she had not obtained Child A's immunization record.

Plan of Correction: The provider will talk to the parent of child today at pick-up time and ensure the immunization record is obtained as soon as possible.
