Discussed with the provider the need to review the Licensing Standards and that those standards must be followed no matter how few children that you have in care. Please remember to post the information form each licensing inspection.
An unannounced Renewal inspection was conducted beginning at 8:50 and ending at 10:15. There were two infants in care this morning with the provider. The babies were observed being given bottles, interacting with the provider and playing on the floor and in infant seat and other play spaces. Standard #: 22VAC40-111-830-A Description: Based on interview with the provider and a review of the Emergency Escape drill form, it was determined that the provider has not been doing monthly emergency drills. Evidence; The provider last did an emergency drill in November. No drills were done in December and January. Plan of Correction: The provider will do an emergency escape drill every month and a shelter in place drill once a year.
Standard #: 22VAC40-111-880 Description: Based on observation and interview with the provider, it was determined that the menu for the babies is not posted. Evidence: Although the provider has USDA menu forms, they are not posted. Plan of Correction: Provider will post the forms every week.
Standard #: 22VAC40-111-960-E Description: Based on observation, it was determined that the provider has not labeled the bottles with the name and the date. Evidence: There are two infants in care. Their bottles are not labeled with the name and date. Plan of Correction: The provider will label the bottles with the child's name and the date.
Standard #: 22VAC40-111-50-A Description: Based on observation and interview with the provider it was determined that there is no daily attendance kept. Evidence: Provider stated that she only keeps USDA attendance. Plan of Correction: Provider will keep attendance daily when children arrive and then cross off when they leave.
Standard #: 22VAC40-111-60-B Description: Based on a review of the records for the two infants in care, it was determined that the files were missing 1 required item. Evidence: Neither child had the Liability Information form in their record. Plan of Correction: The provider will have the parents sign the form tonight.
Standard #: 22VAC40-111-140-B-3 Description: Based on a review of the last CPR card, it was determined that the certification is not current. Evidence: The CPR Card expired 1/20/19 Plan of Correction: The provider will have a new class as soon as possible.
Standard #: 22VAC40-111-140-B-4 Description: Based on a review of the First Aid documentation, it was determined that the provider's First Aid Training was not current. Evidence: The providers First Aid training expited on 1/20/19 Plan of Correction: The provider will get a new class as soon as possible but no later than the end of the month.
