An unannounced monitoring inspection was conducted on February 11, 2019 to determine compliance with family day home standards. The inspection was initiated at 10:00 am and concluded at 12:30 pm. Upon the inspector's arrival four children were in care and playing with toy cars, in the playhouse and with puzzles. A value of 9 point was calculated for all four children with one provider. Later children sat with the provider and sang the song "five little ducks". Hand washing was conducted before lunch and lunch was observed. Today's lunch consisted of salisbury steak, peas, oranges, bread and milk. The main area used for child care was inspected. The outdoor play area was not inspected due to inclement weather. The family day home is equipped with supplies and age appropriate materials. The provider had the following posted: emergency phone numbers, menu, attendance, last inspection notice and the license. The first aid kit and non-medical emergency supplies were checked and found complete. An evacuation drill was documented on 01/29/2019, an shelter-in-place drill on 01/29/2019 and a lock down drill on 01/29/2019. The inspector interacted with the provider and children in care. Four children's records and two caregiver's records were reviewed. Currently there is no medication being administered. No transportation provided. Standard #: 22VAC40-111-800-B Description: Based on review of the emergency preparedness plan, the provider did not document in writing each review and update to the emergency plan annually. Evidence: 1. Documentation indicated the most recent review of the emergency preparedness plan was on 01/09/2018, exceeding the annual requirement. 2. The provider reported the emergency preparedness plan had been reviewed however she did not document the review in writing. Plan of Correction: Per the provider: I will review the plan and document it tonight.
Standard #: 22VAC40-111-100-A Description: Based on interview, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. The provider reported child #3's first day of attendance was 01/10/2019. 2. There was no documentation of a physical examination for child #3 exceeding 30 days after the first day of attendance. Plan of Correction: Per the provider: I will have the parent provide documentation for the child's physical for the record.
Standard #: 22VAC40-111-60-A Description: Based on interview, the provider did not maintain an up-to-date record at the family day home for each child enrolled. Evidence: 1. The provider reported she had not maintained a record for child #3, who re-enrolled 01/10/2019. 2. Child #3 was present during the inspection on February 11, 2019. Plan of Correction: Per the provider: I will have the parent complete a new record for the child tonight at pick up time.
Standard #: 22VAC40-111-90-A Description: Based on interview, the provider did not obtain documentation of immunizations before a child's attendance. Evidence: 1. The provider reported child #3's first day of attendance was 01/10/2019. 2. There was no documentation of immunizations for child #3. Plan of Correction: Per the provider: I will have the parent provide documentation for the child's immunizations for the record.
