Technical Assistance:
*Fingerprint results were due no later than September 30, 2018. Fingerprint results, the search of the central registry (CPS check), and sworn disclosure statement are valid for five years from the original date conducted. ** If Household member 1 is needed as an assistant, Virginia Preservice Training and first aid and CPR certification will be required. *** If oranges and milk are provided at lunch, please find an alternative snack for this afternoon.

An unannounced monitoring inspection was conducted with one caregiver on February 11, 2019 to determine compliance with the licensing standards. There were seven children present, ranging in age from 4 months to 4 years old, totalling 16 points. There was an additional household member present to assist if needed. The children were having rest time. One infant was awake, playing, and being fed a bottle. Seven children's files and three caregiver/household files were reviewed - please see Supplemental Summary for missing items. Caregivers completed first aid and CPR on 11/11/2018. Provider does not administer medication - Medication Administration Training is required to give medicine. Evacuation drills have been conducted monthly. Description: Based on review of seven children's records, the provider failed to have all documentation required in each file. Evidence: Child 4 did not have the name, phone number, and address of a second emergency contact (can not be a parent). Child 5, Child 6, and Child 7 did not have documentation of the health insurance policy number.

Plan of Correction: Provider will request the information from the parent.

Standard #: 22VAC40-111-70-A
Description: Based on review of seven children's records, the facility failed to have documentation all written information was provided to parents prior to the child's first day of attendance. Evidence: the VDSS model form titled "Policy on the Administration of Medication" was in the file, but was not signed by parent of Child 5, Child 6, and Child 7.

Plan of Correction: Provider will request the parent's signature acknowledging the parent is aware of the medication policy.

Standard #: 63.2-1720.1-B-2
Description: Based on review of three household records, the provider failed to submit to fingerprinting prior to the September, 30, 2018 deadline, as required for all providers, caregivers, and household members over the age of 18. Evidence: Two caregiver and one household member's fingerprint results were completed in December 2018.

Plan of Correction: Fingerprint results have been conducted.
