Technical Assistance:
Clarification provided regarding exemption of immunizations.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 6 children (15pts) with Provider and 1 assistant present during the inspection. When the inspector arrived, the children were sitting and having lunch. Lunch today was chicken, broccoli, macaroni and cheese. Staff were observed assisting with feeding the children and also encouraging the older children to eat. A nice transition from lunch to bathroom and hand washing was observed. Staff were engaged with the children on the floor and playing with them. Walk thru of entire programming area was made. Observation of all cribs and the areas around the cribs was made. A total of 4 children's records, and all staff and 1 household record were reviewed. No medication on site as this provider does not administer medications. The home was equipped with ample supplies, toys and equipment age appropriate for the children currently in care. Previous violations from the August inspection was made. One repeated/uncorrected violation found along with one other violation today. This inspection began at 11am and ended at 12:35pm. Thank you for your cooperation today. Standard #: 22VAC40-111-60-A
Description: Based on review of children's records, one record was incomplete. Evidence: Child #2's record is missing authorization for emergency care and proof of identity. This is repeated from the August inspection.

Plan of Correction: I will obtain both of these today from the parent.

Standard #: 22VAC40-111-250-A
Description: Based on observation of the crib room, a hanging item was found within reach of children. Evidence: Window blind cords were found within reach of children in the crib room.

Plan of Correction: Provider tied the cords up during the inspection.
